A1C is glycated hemoglobin, hemoglobin bound to glucose in the circulation. Most red blood cells include hemoglobin. The A1C level is created when red blood cells and sugar interact in the bloodstream. Higher blood glucose levels bind more glucose to hemoglobin, increasing A1C levels. A1C measures hemoglobin linked to glucose over time.
Hemoglobin is a protein found in red blood cells that transports oxygen. Glucose levels rise, sugar molecules bind to hemoglobin, causing anemia. Once glycated, hemoglobin remains in the blood until its red blood cell is removed. A red blood cell's lifespan is 3 to 4 months, therefore the A1C test measures blood glucose during that time. A1C results are a percentage. If your percentage is above the national average, your blood sugar is higher. Normal A1C is below 5.7%. You have diabetes if your A1C is 6.5 percent or above on two tests. A hemoglobin A1C between 5.7% and 6.4% implies prediabetes and increased risk for type 2 diabetes and heart disease. If your A1C is above the recommended range, your doctor will work with you to make lifestyle changes or prescribe medication.
People with diabetes commonly use finger-stick tests and portable glucose meters or continuous glucose monitoring devices such as Dexcom or libre at home. The A1C test measures diabetes management over two to three months. It helps determine how successfully diabetes is managed. A1C readings are given as a percentage or estimated average glucose (eAG) (estimated average glucose level). To calculate eAG, convert A1C to mg/dL or mmol/L. An A1C of 7% equates to an eAG of 154 mg/dL (8.6 mmol/L), while 9% corresponds to 197 mg/dL (11.0 mmol/L) of diabetes. Knowing your eAG can help you put your A1C in context and comprehend how your daily blood sugar levels relate to your overall glycemic control.
