The pharmacy services series looks at the NHS and non-NHS services provided in retail community pharmacies in England and Wales. The Discharge Medicines service in England is an essential service that looks at pharmacys role in helping prevent readmission to hospitals for patients. The Discharge Medicines Review is a similar service in Wales. When patients are discharged from hospital back to the community, the pharmacy team have a role in making sure that the medicines are reconciled and any issues with their return to community is communicated with the hospital or GP surgery as necessary.

This learning is ideal for pharmacy students, pharmacists, trainee pharmacists (previously known as pre-reg trainees) or anyone working in or interested in community pharmacy in Wales and England, to get an understanding of how pharmacy operates.

This series tries to show the value of community pharmacy and clear up misconceptions about the services it provides. It tries to explain how pharmacys service in the community is essential in reducing risks to the patient. I try to facilitate learning with a more animated and dynamic approach to hopefully make pharmacy concepts easier to understand.
