Autopilot nearly killed Elon Musk on his way to work. Faded lane markings caused the system to veer out of the lane, almost hitting ongoing traffic. The engineers couldn't solve the problem quickly, so they debated closing the highway and repainting the lane markings themselves. Luckily, it didn't come that far.
Also, since then, Tesla's Autopilot has been improved massively and should be able to handle faded lines with ease.
