Once your carrier approved your unlock request they will send you an unlock code. In order to input the unlock code you need to be on the unlock code menu page on your phone, just insert a SIM card from a different carrier that the one that locked your phone, and you will be able to input the unlocked code provided by your carrier. Usually you have 10 tries to input the unlock code. If you failed to input the correct unlock code 10 times, then your phone will be "Hard Locked", meaning it will be permanently locked to your carrier.
