Steps to setup custom cover art to your Spotify playlist on PC.

1. Go to the Spotify Playlist on PC - https://open.spotify.com/playlist/
2. Click on the cover art (Playlist picture)
3. Upload image
4. See the change on your Mobile phone too.

Note:
Image should be 300x300 minimum
You can't change Album art on your Mobile, change it on PC to see the Album art on Spotify Mobile.

Changing the Spotify Album art on iPhone or Android Is not possible at this time, you can do that on your PC and see the change on Mobile.
