Alexandra Hospital, Singapore's first Integrated General Hospital (IGH)
AH is Singapore's first public hospital to roll out the IGH model where medical resources move, but the patient doesn't. It shortens hospital stay, has care consolidated by only one principal doctor for all his multiple conditions, through a generalist-led, specialist-supported approach, and reduces number of outpatient appointments. In the wards, it integrates care across acuity from acute to sub-acute and rehabilitation, with optimal right-resourcing and minimal out-transfers. The model minimizes transfer to other step-down care, optimizes care resources, reduces cost, time and reduces hassle for the patient.
The model aims to address the challenges of an aging population and growing chronic diseases burden by providing coordinated and holistic care. It also focuses on empowering patients and actively engaging them in their care.
