I've figured out why I've been getting massive lag spikes. It's a sound issue, and it disappears if I turn the sound off.
Google "minecraft openal" to see what other people have said about the issue.
The quick fix is to find your Minecraft folder, go to .minecraft\bin\natives and find openal32.dll and openal64.dll, and rename them. This only works if your system has it's own version of these files as a part of its audio drivers. If it does, then Minecraft will use those instead, which are better than the ones that come with Minecraft.
If you're looking to fix your lag, just make sure that this is your problem and not something else, and make sure that you understanding what you're doing before you start renaming system files on your computer.
